Professor Mary Gospodarowicz OC is one of the world’s most distinguished radiation oncologists and a towering figure in global cancer control. Born in Gdańsk, Poland, she immigrated to Canada and graduated from the University of Toronto Faculty of Medicine, going on to spend her entire career at Princess Margaret Cancer Centre and the University of Toronto, where she holds the rank of University Professor — the institution’s highest academic designation. Over a career spanning more than four decades, Prof. Gospodarowicz has made transformative contributions to cancer staging through her leadership of the UICC TNM classification system, shaped clinical trial methodology in radiation oncology, and championed global equitable access to radiotherapy. In 2012 she became the first Canadian and the first woman to serve as President of the Union for International Cancer Control (UICC). She is a recipient of the Order of Canada, the ASTRO Gold Medal, the ESTRO Lifetime Achievement Award, and many other international honors that reflect her impact across clinical care, research, education, and global health leadership.

Research Activity

  • Cancer staging and the TNM classification: The defining work of Prof. Gospodarowicz’s career. She has led the UICC’s Prognostic Factors and TNM Process Task Force for decades, overseeing the development and continuous improvement of the international TNM Classification of Malignant Tumours across multiple editions (6th, 7th, 8th). The 7th edition (Sobin, Gospodarowicz, Wittekind, 2009) has accumulated over 28,000 citations, making it one of the most highly cited works in oncology. Her work on the TNM process has been foundational to international consistency in cancer staging and prognostication.
  • Prognostic factors in cancer: Co-editor of the landmark Prognostic Factors in Cancer reference work; led the UICC framework for classifying and applying prognostic factors in clinical oncology; co-authored foundational papers on prognostic factor taxonomy, patient management scenarios, and the role of staging in evidence-based medicine.
  • Genitourinary oncology: Extensive clinical research in testicular cancer (seminoma and non-seminomatous germ cell tumors), bladder cancer, and prostate cancer. Pioneer of surveillance strategies for stage I testicular seminoma; principal investigator on landmark NCIC phase III trials including PR.3 (hormone therapy versus hormones plus pelvic RT in locally advanced prostate cancer) and PR.7 (intermittent vs. continuous androgen suppression), both published in The Lancet and New England Journal of Medicine.
  • Lymphoma: Prolific research on extranodal lymphomas in collaboration with the International Extranodal Lymphoma Study Group; contributed to landmark studies of primary testicular lymphoma, MALT lymphoma, Hodgkin lymphoma treatment and second cancers, and gastrointestinal lymphoma.

  • Image-guided radiotherapy (IGRT): Pioneer of image-guided precision radiotherapy at Princess Margaret Hospital; contributed to foundational studies on cone-beam CT, prostate localization, and dose escalation using IGRT in prostate cancer.
